2. Complete Abstract Form A, including 200-300 word abstract

Integrated Paper Sessions, Symposia, Case Discussions, and Roundtables:

2. Complete Abstract Form B, including 150-200 word abstract

3. Include Name, Degree, Email Address, Presentation Title, and Affiliation for each presenter. Include the mailing address of the Chair (where applicable) and corresponding author. If there is more than one author on an individual presentation or paper, please mark who will be presenting.
